blobs in filesystem (vs in sql table)

Assuming I decide to keep my images in the file system (and the associated
text in sql), what's the best way to give access to a sql-based win app?

I can't give direct access to the folder, and can't think of a nice way for
sql server to deliver or accept the images.  Is a web service it?  Any other
possibilities?

I normally use something like an iframe with an img src tag. But this points
back to the location in the file system.
